Applying Causal AI to Detect CKD Earlier, at Scale
Motivus helped a leading lab benefit management company move beyond traditional markers like GFR to identify early-stage Chronic Kidney Disease across 90 million members by using causal AI to surface clinician-trustworthy, interpretable risk signals.
Building a Diagnostic Intelligence Capability From the Data Up
The company operates a Diagnostic Insights platform across multiple health plans, managing high-volume HL7 lab records for 90+ million members. They needed to go beyond correlational analytics – turning their raw laboratory data into a proactive, early-warning system for Chronic Kidney Disease that clinicians could trust and act on. Motivus built the causal AI engine, data pipelines, and governance model to make that possible.
From Raw Lab Data to Clinically Credible Risk Signals
CKD affects 37 million U.S. adults but often goes undetected until late stages, due to which treatment options get narrow and costs escalate. They had the data, but traditional markers like GFR alone were not enough. The challenge was converting heterogeneous, longitudinal HL7 records into interpretable causal signals that physicians would act on without compromising privacy, auditability, or clinical trust.
What We Did
Built scalable AWS Glue and Databricks pipelines to extract, normalize, and structure HL7-derived clinical signals from fragmented, heterogeneous sources.
Developed causal models using Causalnex and Bayesian networks (grounded in Judea Pearl's causal inference framework) to uncover true drivers of CKD progression beyond correlations.
Engineered time-aware features capturing lab value trajectories and state changes, distinguishing drivers, mediators, and proxies to reduce confounding in the model.
Shaped model outputs into clinician-usable risk signals, enabling proactive screening workflows and physician alerts with transparent, driver-level explanations.
$7M-$11M in New Diagnostic Revenue and Earlier Detection
This leading healthcare intelligence company gained a credible, scalable capability to detect CKD earlier with interpretable causal drivers that clinicians can understand and act on. The engagement opened new diagnostic service revenue streams in the $777M CKD diagnostics market, improved patient outcomes through earlier intervention, and established a reusable causal AI foundation that can extend across its full platform. Designed for 90M+ member scale with repeatable data preparation patterns and HIPAA-aligned governance throughout.
